Hospital group says 'alarm fatigue' can be deadly

CHICAGO (AP) ? Constantly beeping alarms from devices that monitor the vital signs of the critically ill have "desensitized" hospital workers who sometimes ignore the noise, leading to at least two dozen deaths a year on average, a hospital accrediting group said Monday.

And these cases are probably vastly underreported, said the Joint Commission in an alert to hospitals calling attention to the problem.

The beeping devices include those that measure blood pressure and heart rate among other things. Some beep when there's an emergency, and some beep when they're not working. That can lead to noise fatigue and the delay in treating a patient can endanger lives, the accreditation commission says.

Complicating the situation is the abundance of technology, with no standardization for what the beeps mean, said Dr. Ana McKee, the commission's executive vice president and chief medical officer.

The commission's estimate of possible deaths related to the problem is considerably lower than the reports it found in a U.S. Food and drug Administration database. The FDA lists more than 500 deaths potentially linked with hospital alarms between January 2005 and June 2010. But that includes mandatory reports of malfunctions and in some cases the connection to a death is only tenuous.

The commission's own database reports 80 deaths and 13 severe injuries between January 2009 and June 2012. Hospitals voluntarily report these to the commission, which reviews them and in these cases determined there was a clear connection to the device, said McKee.

There likely are far more problems than have been reported, partly because ignoring or misinterpreting an alarm may have set off a chain of events that led to an injury or death, she explained. But tracing back to that first oversight can be difficult, McKee said.

Alarm-system events included patient falls, delays in treatment and medication errors that resulted in injury or death, the Joint Commission said.

The most common factor was "alarm fatigue." But other problems included misinterpreting alarm signals, too few staffers to respond to alarms, and equipment malfunctions.

"With the proliferation of technology, alarms, and a lack of standardization," it's more challenging for doctors and nurses to respond adequately, McKee said.

The commission said hospital leaders need to address the problem and train staffers in safe alarm management.

The organization accredits more than 10,000 U.S. hospital and health care organizations. Hospitals covet accreditation and following commission advice is key to maintaining it.

McKee said the alert will help raise awareness and lead to hospital changes that may save lives.

World powers expect Iran to be ready for progress at next nuclear talks

Iran nuclear talks resume tomorrow in Kazakhstan, with six world powers expecting Iran to ?engage seriously? over a February proposal to curb its nuclear work, according to a senior US official.

?There has been a very positive line out of Tehran on the talks so far,? said the US official, speaking on condition of anonymity. ?We hope that that positive talk will now be matched with some concrete responses and actions on the Iranian side.?

"The onus is really on Iran to respond to the proposal and tell us where they stand," the American official said.

The two-day talks in the Kazakh city of Almaty will build on a proposal put down by six world powers in late February, also in Almaty. The proposal calls on Iran to strictly limit its most sensitive nuclear work ? uranium enrichment to 20 percent, which is technically not too far from bomb-grade ? in exchange for partial relief from crippling sanctions.

Iran?s chief negotiator hailed that proposal as a potential ?turning point? because it requires both sides to take steps. But after 13.5 hours of subsequent technical talks in Istanbul on March 18, an Iranian source said the offer had ?no balance? because Iran, from its perspective, was required to give up more than it got in return.

The US official said the six nations of the P5+1 group (the US, Russia, China, Britain, France, and Germany) negotiating with Iran ?are in complete agreement that this is a balanced and fair and proportionate step-by-step proposal that asks appropriate things of Iran and offers a proportionate response.?

About Iran?s reaction, the US official said: ?I?m not entirely surprised that Iran?s first response would be to say we want more for doing less.? If Iran ?does not take concrete steps to address the concerns of the international community,? the US official added, then sanctions ?pressure only will increase.?

NUTS AND BOLTS

The P5+1 proposal describes the ?voluntary measures? to be taken by both sides in a six-month timeframe ?with renewal by mutual consent,? according to a version of the proposal seen by The Christian Science Monitor.

Although this version could not be independently verified ? these talks are not public ? its details closely match broader descriptions from anonymous American, European, and Iranian sources in recent weeks.

The current proposal calls on Iran to suspend all enrichment above the 5 percent needed to fuel power reactors; to convert its existing 20 percent enriched uranium into fuel for a research reactor, and export or dilute the rest; and to put its deeply buried Fordow enrichment facility in a ?reduced readiness status without dismantlement.?

The proposal lists a number of specific technical steps to be taken at Fordow, including piping and cable disconnections, vacuum adjustments, and removal of ?feed and withdrawal systems? that have until now enabled Iran?s centrifuges there to spin uranium to 20 percent purity.

Iran would also have to accept ?enhanced? monitoring of its nuclear facilities, including cameras at Fordow to provide ?continuous surveillance with live stream transmission? to the Vienna headquarters of inspectors of the UN International Atomic Energy Agency (IAEA).

In exchange, and ?after IAEA confirmation that Iran has implemented all these measures,? the US and European Union would suspend sanctions on gold and precious metals, and the export of petrochemicals.

The P5+1 would also offer civilian nuclear cooperation, including providing fuel for an aging research reactor in Tehran ? which requires 20 percent enriched uranium for fuel ? as well as IAEA technical help with acquiring a modern research reactor, safety assistance, and supplying of isotopes for nuclear medicine.

The US would further ?license safety-related inspection and repair in Iran for Iranian commercial aircraft? bought years ago from American plane-makers.

P5+1 diplomats have said this ?confidence building measure? is a first step, and this version of the proposal states that ?additional significant steps? taken by Iran will yield ?corresponding steps? from the P5+1. ?In return for further significant action? from Iran, it states, the US and EU would be ?prepared to take comparable action, including proportionate relief of oil sanctions.?

BACKED INTO A CORNER?

Even as talks have resumed ? the Almaty I session in February broke an eight-month diplomatic dry spell ? all sides have continued rhetoric that grates against the other.

Iran?s Supreme Leader Ayatollah Ali Khamenei, for example, has several times in recent weeks listed past grievances against the US, and accused Washington of wanting these talks in order to show Iran?s ?weakness.? He has ruled out direct US-Iran talks for now.

Likewise, the US and Israel continue to state that ?all options are on the table? ? meaning military ones as well ? in their bid to discourage any Iranian rush for a nuclear weapon. Iran publicly rejects nuclear weapons, but has yet to resolve questions with the IAEA about possible past weapons-related work.

A nuclear deal is possible and its contours well known, says Seyed Hossein Mousavian, who was a member of Iran?s nuclear negotiating team a decade ago and is now at Princeton. ?But America should try to [help save] the face of the Iranians, rather than push them into a corner," he adds.

Both sides should define their most important needs and what they can expect at the end of the diplomatic process, says Mousavian. He suggests a two-for-two formula, in which the P5+1 would present two lists: one with every single transparency measure they want Iran to take and a second detailing every measure and guarantee required for Iran to limit enrichment to 5 percent and prevent a ?break out? to a nuclear bomb.

Iran?s two lists would focus on its two priorities: Recognition of its ?right? to enrich uranium, and the lifting of nuclear-related sanctions that have choked its economy. Mutual agreement on such lists, says Mousavian, ?will help both of you know the end state.?

READY FOR PROGRESS

Some of those conclusions are echoed by the ?Iran Task Force? of the Washington-based Atlantic Council, which reports today, after a two-year examination of Iran issues, that the US ?should prepare a roadmap, to be used in negotiations, for gradually removing sanctions as concrete agreements are reached. Iran needs to see off-ramps and an endgame.?

The Atlantic Council report says a US military option is a ?last resort,? but that the Obama administration ?must ensure that this threat remains credible, as it may ultimately be the only course that deters Iran from deciding to build nuclear weapons.?

The report also prominently calls for ?new measures to augment people-to-people ties, support Iran?s democratic evolution, and facilitate trade in food, medicine, and medical supplies? as a ?potent goodwill gesture.?

Those recommendations are among the ?most important,? says Barbara Slavin, a senior fellow at the Atlantic Council who drafted the report.

?The US-Iran dispute has been going on for decades and is not about to end anytime soon. But someday, the US and Iran will have diplomatic relations again and we need to prepare the ground for that now,? says Ms. Slavin.

A first step may be what happens at Almaty II, though diplomats don?t yet know what to expect from Iran, says the senior US official.

?The only thing we?ve heard is ? from the public rhetoric, which has been repeated in private conversations people have had with the Iranians ? those who do have communications with them ? and that is that they?re going to come in a positive frame of mind ready to continue to make progress,? says the US official. ?But whether that is going to be meaningful, we have no idea.?

Another Senate Democrat changes mind on same-sex unions

After years of opposing redefining marriage to include unions between same-sex couples, a number of senators?most recently Illinois Republican Sen. Mark Kirk?have announced that they now support gay marriage.

Kirk, only the second Republican in the chamber to declare his support after Ohio Republican Sen. Rob Portman did so last month, said Tuesday that he now believes that the "government has no place in the middle" of two people who love each other.

Kirk's announcement, made in a statement posted on his website, arrived just hours after Delaware Democratic Sen. Tom Carper wrote on his Facebook page that after "a great deal of soul searching" he had concluded that "all Americans ultimately should be free to marry the people they love and intend to share their lives with, regardless of their sexual orientation."

Just a day before, Pennsylvania Democratic Sen. Bob Casey made his own declaration of support, following the lead of several Senate Democrats who over the last week or so endorsed same-sex marriage.

With Carper and Casey's announcements, only seven Senate Democrats still oppose same-sex marriage: Mark Pryor of Arkansas, Joe Donnelly of Indiana, Bill Nelson of Florida, Joe Manchin of West Virginia, Heidi Heitkamp of North Dakota, Mary Landrieu of Louisiana and Tim Johnson of South Dakota.

Pressure continues to build on the Democratic holdouts?many of whom face re-election battles next year. Liberal advocacy groups like MoveOn.org are currently conducting coordinated campaigns to sway the remaining senators.

Recent national surveys suggest that public opinion is swinging in favor of granting same-sex couples the legal right to marry. A national ABC News-Washington Post poll conducted last month found that 58 percent of Americans now support allowing same-sex marriage. Ten years ago, only 37 percent believed the government should recognize same-sex marriages. Last week the Supreme Court heard two cases on the subject, which addressed the constitutionality of a voter-approved measure that banned same-sex marriages in California and the Defense of Marriage Act.

NASA team investigates complex chemistry at Saturn's moon Titan

Apr. 3, 2013 ? A laboratory experiment at NASA's Jet Propulsion Laboratory, Pasadena, Calif., simulating the atmosphere of Saturn's moon Titan suggests complex organic chemistry that could eventually lead to the building blocks of life extends lower in the atmosphere than previously thought. The results now point out another region on the moon that could brew up prebiotic materials.

The paper was published in Nature Communications this week.

"Scientists previously thought that as we got closer to the surface of Titan, the moon's atmospheric chemistry was basically inert and dull," said Murthy Gudipati, the paper's lead author at JPL. "Our experiment shows that's not true. The same kind of light that drives biological chemistry on Earth's surface could also drive chemistry on Titan, even though Titan receives far less light from the sun and is much colder. Titan is not a sleeping giant in the lower atmosphere, but at least half awake in its chemical activity."

Scientists have known since NASA's Voyager mission flew by the Saturn system in the early 1980s that Titan, Saturn's largest moon, has a thick, hazy atmosphere with hydrocarbons, including methane and ethane. These simple organic molecules can develop into smog-like, airborne molecules with carbon-nitrogen-hydrogen bonds, which astronomer Carl Sagan called "tholins."

"We've known that Titan's upper atmosphere is hospitable to the formation of complex organic molecules," said co-author Mark Allen, principal investigator of the JPL Titan team that is a part of the NASA Astrobiology Institute, headquartered at Ames Research Center, Moffett Field, Calif. "Now we know that sunlight in the Titan lower atmosphere can kick-start more complex organic chemistry in liquids and solids rather than just in gases."

The team examined an ice form of dicyanoacetylene -- a molecule detected on Titan that is related to a compound that turned brown after being exposed to ambient light in Allen's lab 40 years ago.

In this latest experiment, dicyanoacetylene was exposed to laser light at wavelengths as long as 355 nanometers. Light of that wavelength can filter down to Titan's lower atmosphere at a modest intensity, somewhat like the amount of light that comes through protective glasses when Earthlings view a solar eclipse, Gudipati said. The result was the formation of a brownish haze between the two panes of glass containing the experiment, confirming that organic-ice photochemistry at conditions like Titan's lower atmosphere could produce tholins.

The complex organics could coat the "rocks" of water ice at Titan's surface and they could possibly seep through the crust, to a liquid water layer under Titan's surface. In previous laboratory experiments, tholins like these were exposed to liquid water over time and developed into biologically significant molecules, such as amino acids and the nucleotide bases that form RNA.

"These results suggest that the volume of Titan's atmosphere involved in the production of more complex organic chemicals is much larger than previously believed," said Edward Goolish, acting director of NASA's Astrobiology Institute. "This new information makes Titan an even more interesting environment for astrobiological study."

The team included Isabelle Couturier of the University of Provence, Marseille, France; Ronen Jacovi, a NASA postdoctoral fellow from Israel; and Antti Lignell, a Finnish Academy of Science postdoctoral fellow from Helsinki at JPL.

China announces two new bird flu cases, including one death

BEIJING (Reuters) - China has found two more cases of a new strain of bird flu and one of the victims has died, state media said on Wednesday, bringing to nine the number of cases.

A 38-year old cook became ill early last month while working in the province of Jiangsu, where five of the other cases were found. He died in hospital in Hangzhou city on March 27, the Xinhua news agency reported. Samples tested positive for the new H7N9 strain on Wednesday.

The second patient, also in Hangzhou, is a 67-year old who is under treatment. Xinhua said no connection between the two cases had been discovered, and no one in close contact with either patient had developed any flu-like symptoms.

Of the seven other cases of the new strain, two have died, both in the business hub of Shanghai. The other five are in a critical condition in hospital in Nanjing.

Shanghai, Nanjing and Hangzhou are all close to each other in eastern China.

The Agriculture Ministry said it had yet to find any animals infected with H7N9, though added it was possible it had been brought to China by migratory birds.

The World Health Organization said on Monday that the first three cases had shown no evidence of human-to-human transmission, but that there were questions about the source of the infection and the mode of transmission.

China has stepped up its alert level since the cases came to light and has said it is being transparent in dealing with the outbreak.

China has a checkered record when it comes to tackling bad news, which has been known to be covered up by officials fearing it may attract unwanted attention from superiors and damage promotion prospects.

In 2003, authorities initially tried to cover up an epidemic of Severe Acute Respiratory Syndrome, which emerged in China and killed about a tenth of the 8,000 people it infected worldwide.

NASA Climate Scientist James Hansen Quits to Fight Global Warming

Climate scientist James Hansen is retiring from NASA this week to devote himself to the fight against global warming.

Hansen's retirement concludes a 46-year career at NASA's Goddard Institute for Space Studies in New York, but he plans to use his time to take up legal challenges to the federal and state governments over limiting greenhouse gas emissions.

In recent years, Hansen, 72, has become an activist for climate change, which didn't sit well with NASA headquarters in Washington. "As a government employee, you can't testify against the government," Hansen told The New York Times.

Supporting his "moral obligation" to step up to the fight now, Hansen adds in the Times article that burning a substantial fraction of Earth's fossil fuels guarantees "unstoppable changes" in the planet's climate, leaving an unfixable problem for future generations.

The distinguished NASA scientist has spent his career at the Goddard Institute on the campus of Columbia University. He has testified in Congress dozens of times, and has issued warnings and published papers that drew criticism from climate-change skeptics. [The Reality of Climate Change: 10 Myths Busted]

Hansen was arrested in February while protesting the proposed construction of the Keystone XL Pipeline that would carry heavy crude oil from Canada to the U.S. Gulf Coast. "We have reached a fork in the road," he told the Washington Post at the time, adding that politicians must understand they can "go down this road of exploiting every fossil fuel we have ? tar sands, tar shale, off-shore drilling in the Arctic ? but the science tells us we can't do that without creating a situation where our children and grandchildren will have no control over, which is the climate system."

With his departure from NASA, Hansen told the Times he plans to lobby European leaders to institute a tax on oil derived from tar sands, whose extraction leads to more greenhouse gas emissions than conventional oil. He could not have done these things as a government employee, he said.

Hansen will probably work in a converted barn on his farm in Pennsylvania, but may possibly set up a small institute or take an academic appointment, according to the Times. He will continue to publish papers in academic journals, but will not run the powerful computers and other resources NASA provided for tracking and forecasting global warming and its effects.

Raised in a small town in Iowa, Hansen initially studied the planet Venus, but switched to studying the effect of human greenhouse gas emissions on Earth during the 1970s.?

He was one of the first scientists to raise alarm about global warming and its effects on climate and the environment. After testifying at a Congressional committee in 1988?that man-made global warming has begun, Hansen was quoted widely as saying, "It is time to stop waffling so much and say that the evidence is pretty strong that the greenhouse effect is here."

Hansed joined NASA's Goddard Institute as a post-doctoral scholar in 1967 and became a federal employee in 1972. He became director in 1981, and was?the longest-serving director in the intistute's history. "He has pushed forward the frontier of our knowledge of Earth's climate system and of the impacts that humanity is having on Earth?s climate," Nicholas E. White, director of the Sciences and Exploration Directorate at Goddard, said in a statement.

Climate scientists applaud Hansen for leading the predictions of climate change's effects. But some say these predictions were exaggerated. For example, he has said in recent years that vast carbon dioxide emissions might ultimately cause a runaway greenhouse effect like on Venus that would boil the oceans and make Earth uninhabitable, the Times reported. Other scientists say this hasn't happened in the past and that Hansen overstated the risk.

Hansen was embroiled in a political fight in 2005, when a young political appointee in George W. Bush's administration tried to muzzle Hansen in the press. But Hansen revealed this to the public in an interview reported by the Times, and the administration lifted its restrictions.

Despite his environmentalist stance, Hansen has also criticized the environmentalist movement. He strongly opposed a failed climate bill in 2009, because he said it would have given the federal government billions of dollars without truly limiting emissions.

Hansen, who is registered as an independent, believes carbon dioxide emissions should be taxed, but that the money should be returned to the public as a rebate, instead of going to the goverment.

Hansen told the Times he senses a mass movement on climate change is beginning, led by young people, which he plans to support.

Penney CEO gets no bonus, stock award after dismal year

(Reuters) - J.C. Penney Co Inc Chief Executive Ron Johnson received total compensation of $1.9 million in 2012, according to a regulatory filing on Tuesday, a sharp decline after a year that saw the department store chain's sales fall perilously.

Johnson, who oversaw the development of Apple Inc's retail business before becoming Penney's CEO in late 2011, unveiled a plan last year to reinvent the 111-year-old retailer. The plan included the elimination of most discounting, a move that contributed to a 25 percent drop in sales last year.

Johnson's base salary was unchanged at $1.5 million, but he received no bonus or stock award in 2012 because of Penney's results, Penney said in its filing. He was eligible in 2012 for a bonus of $1.875 million.

Last year, his total compensation included a stock award worth $52.7 million to make up for incentives he was giving up by leaving Apple.

Johnson's 2012 compensation included $344,213 for personal use of a corporate jet. Johnson commutes to Penney's Plano, Texas, headquarters every week from his home in California. That amount is similar to the personal use of a corporate jet by Johnson's predecessor Myron Ullman in 2011.

A source told Reuters in April that, after a disastrous first year for Johnson's turnaround, the executive might only have a few more months to carry out his vision of turning Penney stores into emporia offering trendy but low-price items.

If Johnson quits or is fired, he will not get a golden parachute. He would only receive $5,000 and restricted stock worth $106,424 as part of a package worth $148,924 in all, according to a proxy statement filed with the U.S. Securities and Exchange Commission.

In his annual letter to stockholders, Penney Chairman Thomas Engibous limited himself to inviting them to Penney's annual meeting in Plano on May 17, in contrast to a seven-paragraph note last year touting Johnson's turnaround plan.

Penney's board renominated all 11 of its current directors for another one-year term, choosing not to fill a spot vacated by the death in August of longtime board member Burl Osborne.

None of Penney's executive officers got any 2012 cash bonuses. Former Penney President Michael Francis, who left in June after only 9 months with the company, was given a termination fee of $3.6 million.

Penney shares slid 44 percent in 2012 and have fallen 26 percent so far in 2013.

NEC looking to hawk mobile division to Lenovo, says Bloomberg

Though we don't get to see its smartphone wares too often stateside or in Europe, NEC has always said its mobile division was a big part of its business. Now it looks to be trying to fob that arm off to PC venture partner Lenovo, according to unnamed Bloomberg sources. The Japanese company is also said to be eying potential domestic buyers, and Reuters recently reported that it's selling retail subsidiary NEC Mobiling to the tune of $850 million. The move is said to be in the works to bolster profitability after two straight years of smartphone operation losses and 10,000 layoffs, but as always, such unattributed material needs to be digested with beaucoup salt.

Researchers at Moffitt Cancer Center and colleagues at the University of Florida studied health care providers to determine the factors associated with disparities in Human Papillomavirus (HPV) vaccination among girls, ages 9 to 17, from low-income families. They found that physician vaccination strategies and the type of practice play a role in whether or not girls were vaccinated.

The study results were published in the Feb. 1 issue of Cancer.

"The HPV vaccine has made cervical cancer preventable. However, low-income and minority women still experience a higher number of deaths from the disease," said Susan T. Vadaparampil, Ph.D., M.P.H., member of the Health Outcomes and Behavior Program at Moffitt. "There are no-cost HPV vaccination programs available, such as the Vaccines for Children program, to help, but we still find low-income and minority females are less likely to receive the vaccination series."

The HPV vaccine has been recommended for females ages 11 to 26 since 2006. Despite the availability of the vaccine, more than 12,700 women were diagnosed with cervical cancer and 4,290 died from the disease in 2011. As a result, the federal government has included the reduction of HPV infection rates and an increase in HPV vaccination rates in its Healthy People 2020 program.

In order to assess the factors associated with low HPV vaccine rates, the researchers surveyed a random sample of 800 Florida health care providers from the Medicaid Master Provider File. The researchers mailed the providers a 27-item survey that evaluated the characteristics of their practice, their HPV information and knowledge, possible barriers to HPV vaccination, and their vaccine recommendation and strategy practices.

The researchers found that:

  • Family physicians had a lower patient HPV vaccination prevalence than pediatricians.
  • Physicians in multi-specialty practice were less likely to vaccinate than those in single specialty practice.
  • Providers using two or more strategies to ensure HPV vaccination were more likely to vaccinate than those with no strategies.

The researchers also found that providers of the no-cost Vaccine for Children program were more likely to vaccinate than those who did not participate in the program. Also, physicians who reported caring for minority groups had higher vaccination averages than those caring for non-minorities.

"This research helps us identify key ways in which we can target future provider interventions," Vadaparampil said. "Our next steps will be to develop interventions that target the providers and settings where vaccination rates were lowest."

Given that black and Hispanic women have higher rates of cervical cancer when compared to their white counterparts, these physicians may be more sensitized to the importance of vaccination against cervical cancer, said the researchers.

The finding they called "interesting" was that vaccination rates were relatively low in light of a patient's eligibility for free vaccination through the Vaccine for Children program. Other factors may have impeded vaccination, they suggested, such as disruption of provider vaccination supply or patient refusal.

"Since all patients of all physicians surveyed were eligible for free vaccination due to their participation in the Florida Medicaid program, our findings suggest that cost may not be the only barrier to vaccination," explained Teri Malo, Ph.D., M.P.H., applied research scientist in the Health Outcomes and Behavior Program at Moffitt.

"Our findings suggest that HPV vaccination disparities among low-income females, even in the absence of cost, require targeted intervention to increase vaccination rates among underserved populations," concluded the researchers. The intervention may focus on family physicians and specialty physicians who practice in non-private and multi-specialty settings who see primarily non-Hispanic white patients and use no strategies to ensure vaccination completion.

If you thought Evernote, the multipurpose note-taking and organization program, couldn't get any more useful, wait until you get a look at all the new bonuses in version 5 of its Android app (free). This latest release overhauls the camera feature used for taking photos and saving them into your cloud-based Evernote account to make it more efficient, and adds shortcuts to frequently accessed notebooks, tags, and search terms via right-to-left swipe gesture. It also adds some really exciting new possibilities for what you can do with the Evernote Android app in conjunction with paper notebooks, the kind you'd use to keep sketches and handwritten notes. Online meets offline?what a twist. Evernote's Android app goes above and beyond what any mobile user could reasonably expect it to do, earning it five out of five stars and PCMag's Editors' Choice.

For the uninitiated, here's the 30-second explanation of what Evernote is: It's a freemium note-taking program that stores all your notes in the cloud so that you can access and edit them from any nearly place you have an Internet connection. You can type notes, upload images as part of your notes (think whiteboards, recipe cards, phone numbers scribbled on napkins), and record voice memos. Metadata, such as tags and your geographic location when you wrote the note, are stored with it, too, giving you a dozen or more ways to search for your notes later when you need them. The free version gives you a lot?there are no major shortcomings to working with it?but the $45-per-year Premium account is a sweet deal (more on the differences below). You can create Evernote notes in the desktop app for Windows or Mac, through the Evernote Web account, via its mobile app for iPhone or iPad, and of course in the Android app, which is the focus of this review here.

In many ways, Evernote is similar to Microsoft OneNote (which is part of SkyDrive), although on Android devices, it's more comparable with the newly released Google Keep Android app (free, 3 stars). Evernote is a clear head above Keep, in part because it works so well in all its various forms, whereas Google's Keep online version (free, 2.5 stars) is but a pitifully lightweight program with obvious bugs. Head-to-head against Evernote, OneNote pales in comparison, especially in terms of how and how well it helps you organize, find, and edit your notes. More importantly, Evernote integrates with dozens of other third-party tools, not to mention other useful apps and plug-ins under the Evernote umbrella (for example, Evernote Web clipper is a browser plug-in that quickly saves the contents of a Web page without making you copy and paste anything). The service just can't be beat.

What's New in Evernote's Android App Version 5.0?
The noteworthy features in Evernote's latest Android release are many, but my favorite is a new shortcut menu, which appears when you swipe from right to left. Shortcuts bring you to a search result of your choosing in just one swipe and one tap. For example, you can set up shortcuts to particular notebooks or tags. On a small smartphone screen, having these shortcuts makes a lot more sense than scanning through a scrolling list of all the available options.

Another new feature in version 5 of the Evernote Android app has to do with the camera features. When you snap a photo to upload now, you can shoot multiple photos at once and select the ones that you want to include after their thumbnails are on screen. It's a great time saving feature for when you're taking multiple photos of pages of a document or book.

The special twist in this release has to do with how Evernote plays nice with other apps, in this case, real paper notebooks (and more specifically, the Moleskine brand). You can now buy special Evernote stickers (included with select Moleskine notebooks, starting around $25) that you apply to real paper and that turn into tags when you upload an image of that paper to Evernote. And you can customize the sticker's meaning. For example, one of the stickers is a blue square with a white airplane. You can customize your Evernote Android app in the settings such that this sticker adds a tag called "travel" and automatically saves any images that use this sticker into a notebook called "Itineraries." It's all highly customizable, and quite frankly, pretty darn cool.

Another new feature is voice-to-text speech. In other words, you can now dictate notes, and Evernote will save an audio recording of your voice memo, too.

Back to Basics
As in all the other versions of Evernote, the Android app gives you access to a lovely set of organizational tools: tags, notebooks, and stacks, which are groups of notebooks. These tools hide behind the left side of the screen, but are laid out spaciously here and within their sub-pages in a way that makes them easy to access even on a small screen.

Within your own tags and notebooks, you can click on any one to show content that meets your search criteria. An additional text search bar, found through the eyeglass icon, gives you additional ways to get to the notes you need. One very cool feature that? isn't new (I still love it though) is that if you have any uploaded images that contain the keywords from a search?even if they are handwritten on a scrap of paper or whiteboard?Evernote will find them. In the desktop and Web apps, the matched words are highlight in yellow, although they're not called to attention in the Android version. This OCR feature isn't foolproof, but it's an incredible addition that's available in both free and Premium accounts.

Evernote Premium vs. Free
Evernote Premium costs $5 per month or $45 per year and adds some noticeable perks to the free Evernote account. Free account holders can only upload 60MB of data per month, whereas paid users max out at 1GB per month, which comes in handy if you upload a lot of images and documents or clip a lot of Web pages. When Premium users enter search terms, Evernote not only scans typed content and images, but also hunts through PDFs.

Another perk for Premium users is offline notebooks, an amazingly useful option when working on a mobile device. This feature lets you save the most recent copy of selected notes locally on your Android so they're available to read and edit when you don't have an Internet connection.

Premium users can give other users the ability to edit a note, adding collaboration to the experience. When you decide to share a notebook, you can to make the files viewable only, or both viewable and editable. You can require that they be an Evernote account holder or not. When you're ready to share, one click sends a short email with a link to the files. (For additional details about Evernote Premium accounts, see Evernote's information page).

Forever Note
You can do a lot with the free version of Evernote, and I would only recommend upgrading to Premium if you're so committed to Evernote that it will be your one-and-only note-making app. Luckily, committing to Evernote is not a difficult decision, especially if you're an Android user, as this edition of Evernote is one of the more complex and rich ones available. Most of the new features aren't even available yet in other mobile versions of the Evernote app?sorry, iOS users; you'll have to wait. Evernote is easily the best note-taking service you'll find. It integrates with so many other programs and services, including now actual paper. All the thoughtful features and excellent search tools just make Evernote a stellar service. If you want to get organized, Evernote deserves your attention.?

Microsoft's next Office update is codenamed 'Gemini,' could ship this fall

Blue is to Windows as Gemini is to Office. Any questions? Well, that's unfortunate -- we don't have a ton of answers. What we do know, courtesy of some top-notch ZDNet reporting, is that Microsoft's next Office release is codenamed "Gemini," and that it could be making its way to your desktop by the end of this year. The site quotes a potential fall ship date, at least for the first release, which is set to include new versions of Excel, OneNote, PowerPoint and Word. Rumor has it that the next version will be somewhat platform-independent, and will rely heavily on the Office 365 subscription concept. Beyond that, there isn't really much to share, but you're welcome to hit up our source link for a few more (minor) details.
